I'm not sure. You could try getting tech support on discuss.kde.org or you
could try getting Ubuntu tech support since KDE neon is based on Ubuntu. It
could also be a power draw issue. When I use my 4K portable monitor (different
from the LG one) while traveling, I have to set the resolution to 1920x1080 or
else it will turn off and on repeatedly like in your video.

Not really, or at least I don't know of one. I have seen issues with my
external monitor (LG 27UK650-W) that were similar to yours, but Spectacle
didn't have anything to do with it. It seems like some kind of timing issue,
often involving the screen being turned off in some way before turning it back
on. Examples: unplugging and replugging the external screen, putting my laptop
to sleep and waking it up, letting the external screen go to sleep and waving
the mouse to wake it back up.

I lowered my refresh rate to 60 Hz and it worked. Then I set it back to 144 Hz
and it also works. How strange.

What’s curious is that now KWin doesn’t detect the laptop screen, only the
external one. Also interestingly, it doesn’t set the refresh rate to exactly
144 Hz, but to 143.92 Hz, whereas it always used to show 144 Hz.
