assuming that the first three occurances of blanks can be regarded as
field separators you can do:
prompt> cat file.txt | sed \
-e "s/'/'/g" \
-e "s/ */','/" \
-e "s/ */','/" \
-e "s/ */','/" \
-e "s/^/INSERT INTO categories VALUES('/" \
-e "s/$/');/" | mys
