I think this should happen automatically. i.e. I think you are seeing
a bug. Please give an example of two template calls, one that does not
work with a form and one that does.

Please try copying a template call created with a form to a new page
and see if it gets picked up by the form correctly.

It could be that I'm wrong, but I didn't think that a form was
magically linked to the template calls that it creates. I thought it
just 'picked up' the template call from the page. Or perhaps someone
in the know can tell us if the reported behaviour is actually
expected?
