I don't know if Icedove is tired or if I'm tired, I send it again to
myself ;), but so I can add a note. As far as I remember the two HDA
Intel devices also had consistent indexes, while the alsa-base was set to

alias snd-card-1 snd-hda-intel
options snd-hda-intel index=1
alias snd-card-2 snd-usb-audio
options snd-usb-audio index=2

Maybe the order is done by the IRQ or something else, if more devices need the 
same driver.

Hm, now I also can copy the indexes to alsa-base before I finally will shut 
down the computer for the next hours ... if I'm able to do this ...

64studio-utils (2.1)
alsa-firmware (1.0.15-1~etch1)
alsa-firmware-loaders (1.0.13-1)
backgrounds (0.2)
bootsplash-theme-64studio (0.3)

Installed the following packages:
alsa-base (1.0.16-1~etch1)
alsa-modules-2.6.21-1-multimedia-amd64 (1.0.17~hg080328-1+2.6.21-2)

I have 4 Sound devices.

hwinfo --sound
22: PCI 14.2: 0403 Audio device
  Driver: "HDA Intel"
  IRQ: 16
30: PCI 105.2: 0403 Audio device
  Driver: "HDA Intel"
  IRQ: 19
33: PCI 306.0: 0401 Multimedia audio controller
  Driver: "ICE1712"
  IRQ: 21

And an USB-MIDI. I usually only use the ICE1712 and the USB-MIDI, that's
why I don't care about consistent indexes for the both HDA Intel.
Anyway, after doing all this, all 4 sound devices are available at the
same time again. It might be helpful to make a copy of the alsa-base's
part where the entries for the indexes are, before doing this ;).
