I do not think that the 2.6.21 kernel is new enough for the udev utils
that Ubuntu uses.  Try booting from a Live CD (or the Ubuntu that is
already on the system) and edit the menu.lst

What you could try is replacing the device portion for root=
Replace UUID=354a3759-3e65-4fa9-91f1-3e73b3f0d8a1 with the /dev/sdx that
is appropriate for the actual device.

You can do this temporarily within grub by choosing the edit option and
editing the root= portion.  Then use the onscreen prompts to boot.
