For those who have the ability and the desire to install lilypond(*),
you can get MIDI files from your abc by running abc2ly and then
running ly2dvi on the resulting lilypond. I added limited support for
the Q: field to the version which was released yesterday as
lilypond-1.4.3, and can add other MIDI-related features if there's
demand.
* Requirements include a system running linux (and possibly some
other unices), windows (probably not all varieties), or Mac OSX,
with a fair amount of free disk space and other system resources.
