I've seen issues like this caused by an unreliable network (bad hardware), unexpected server restarts, or three finger salutes (CTRL, ALT & DEL) by end users killing the FE while the . As a previous writer identified, a corrupt form could also cause the issue. You might get a copy of ldbview.exe and see what user is causing the corruption. This application will tell you what user is having or had 'suspect' transactions.
