In fact it may sound bad, but pizzas in western countries are eaten by 
picking them up in both the hands. In standard pizza restaurants, they are 
sliced in triangular shapes. So what one does is to hold two sides with both 
hands at the bottom and taking bites on the third side. As they become 
smaller, one hand is gradually released from the burden of holding them. But 
again the top potion is covered with sticky cheese and other sausages and 
henceforth not to be touched. Although I didn't know this in the beginning 
and I either used to get them packed or up on insistence of my friends make 
a mess out of it. The real challenge is with sandwiches. We in India make 
them in reasonable sizes, but here in the US they stuff too much of meat and 
other vegetables. Thus it becomes really difficult to hold them and 
experiencial learning really is the only way of overcoming it.

I think it is acceptable to eat sandwiches and pizzas by picking them up
with the right hand and biting into them. That is done by westerners too who
otherwise use cutlery all the time.

may be you could role the fork into the noodles and then disentangle them
either with the same hand or use the left if possible. you won't need to
individually use single fingers in this case.

I can easily eat noodles with fork by rolling them into fork and
disentangling using the left hand. or sometimes shaking them with the right
hand itself. extra noodles fall off from the fork by themselves.

as regards the numerous dishes on the plate, I for one prefer not to
overfill my plate;
then I locate vegis one after another as I go about eating. so I don't first
orient myself...instead eat and become oriented on the way. either way I
know what Ihave asked for and it doesn't matter in which sequence I eat
them!

when eating idlies, I cut them with spoon and dip in sambar bowl without
having to use my hand at  all.

of course cutting with knife and then eating with fork is difficult with
respect to sandwitches, pitzas and such stuff.
I prefer to cut longer pieces instead of meticuously small ones and  use
hand after the first bite.

ice creams and custards like soup in a bowl are problematic only sometimes.
eating is not so much a problem as much is the risk of dripping. that's what
I'm bothered about. i prefer smaller quantity in the spoon than the spoon
can take. so lesser chances of dripping on the way.
most of these work well with me.

an important  thing that i never forget to do is to use tissue after every
bite and keep cloth napkin on the lap. I don't mind asking for more tissue
papers when needed!

afterall everyone has learnt from someone  and remember learning comes with
habit!
