Once you are presented that dialog box, using Jaws can you press Insert+q and get the file name which is announced? Insert+Q does announce currently loaded application and script file if any. For instance, if you are in Microsoft outlook it will announce Microsoft Outlook settings are used in the OUTLOOK.EXE application. This will help for further diagnosing which application is producing this dialog box.
