Much depends on what you want to do. If you want to scan and read at the same time,, then Kurzweil or Open book are the way to go. Another advantage of blind specific OCR(s) is that they seem to do a better job of extracting text from graphics in a book.
As for the commertial Ocrs, Fine Reader v11 works nicely with screen readers. Omnipage may do so too.
