Hi, What version of dragon are you using? The latest is version 12.5. What screen reader are you using? If jaws, you may want to procure J-Say which is middleware that interfaces jaws with dragon.
You can use Dragon without a screen reader. What microphone and soundcard do you have? Using speech-recognition is a non-trivyal activity. What are your specific problems?
