I use the -mm patch all the time, and it generally works fine. 
So I suspect you're applying it against the wrong tree.  For 
2.6.16-rc5-mm1, you should: 
 
  - unpack a 2.6.15 tree 
  - apply the 2.6.16-rc5 patch 
  - apply the 2.6.16-rc5-mm1 patch 
 
I use "ketchup," which does all this stuff automatically: 
 
  http://www.selenic.com/ketchup/wiki/ 
 
For example: 
 
  $ mkdir work-mm 
  $ cd work-mm 
  $ ketchup 2.6.16-rc5-mm1
