Current versions of Ntbackup, Backupexec and Retrospect can all do backups to a disk file. That doesn't mean it is a "flat file backup". They all use the Exchange APIs to do it, they just store the data to a disk file.
Doing an incremental or differential of an Exchange storage group during the day is rather like doing SQL backups of transaction logs - it can get you closer to a point-in-time recovery in the case of a disaster. I've used them in a lab situation. Never needed one in a live recovery. I know very few clients that do this. I can imagine that some businesses might require it.
