Hi Scott,

At Penn State, I'm the policy facilitator who works with our "policy stewards." 
 We have a policy steward for each of our 10 major policy categories, ranging 
from Administrative Policies to Travel Policies.  After a thorough vetting 
process, I solicit the approval of the policy steward.  At the bottom of our 
policies, we list the following:

Effective Date: December xx, xxxx
Date Approved: December xx, xxxx
Date Published: December xx, xxxx (Editorial changes, March 17, 2015)

In our system, the effective date is the key one.  In most cases, the 
Effective, Approved and Published dates are the same, but not always, for a 
variety of reasons.  But the effective date is typically the "official" date 
the policy goes into effect.

If minor, editorial changes become necessary, I will make the change and note 
it, as stated above (typos, a title change, etc).  But the effective date is 
unchanged.  If MAJOR changes are flagged, then we commence with another policy 
review process, involving the policy steward and the appropriate policy review 
team.

Colleagues,

The University of Illinois is reviewing lifecycle dates associated with our 
business and financial policies and procedures. Each policy currently has 
"approved on" and "last updated" dates listed. There has been some confusion 
regarding if the "approved on" date, is the same as an effective date.

How do your institution(s) list these dates? Is it part of a standard template? 
Has anyone recently changed or modified the way they list dates based on 
customer feedback?
