I remain absolutely convinced that this is a byproduct of the tetex subsystem. During my maintenance of jadetex I've come across many such problems. I don't really know if there's anything we can do about it.
Someone could probably reproduce the problem by installing slink tetex, configuring it a lot, and then upgrading it to potato and woody.
