I'm running CS5 under Windows 7 64-bit with no audio problems at all.
The sentence you write that bothers me is : "When I create a timeline 
that's not Matrox and switch to Premiere Pro WDM Sound in audio hardware 
preferences (as opposed to ASUS Xonar D2 speakers), I can hear normal 
audio when playing in both the preview monitor and timeline"
That suggests to me that you have a Matrox problem, probably a driver 
issue. I'd suggest that's hwere your problem lies.
