available in CS5.5. Unfortunately it worked briefly, then my screen
went blank. I'll have to track down the bug that's causing that.

There's a quicker way to get that menu - click the icon on the Program 
Monitor (top right) -> Playback settings.

And if you have 'disable video output while in background' ticked, the 
fullscreen image disappears (on purpose) as soon as Premiere looses the 
focus (ie. you click away from it or minimize it).  Untick it to keep it 
always there - can be fun sometimes though when you really need to get 
to something behind it : ).
