Make sure you've registered the 'MMCFormsShim.dll' too.  Other than
that, you should be all set.  I've switched machines a couple of times
recently and created a small batch file to get my MMC add-in set up and
running.  All it has is:

regasm my.dll /codebase
snapreg my.dll
