I've been seeing that too. Currently, we have a campaign we can't complete processing on. We'd be in the middle of processing ad groups and suddenly get a failed ad group creation after successfully creating several ad groups. We've only had this condition occur with the v2009 code. We upgraded to v2.1 and still have the condition.
