After working with Aiki Forms more in-depthly, over the past few weeks,
I feel like I'm getting the handle on the reason for this bug.
I think multiple clicks on the submit buttons are responsible for the
majority of these submits. When an aiki form is given the ':ajax'
argument (as is the case in the comments form), the form hangs while the
data is posted & handled. If a user clicks submit again during this
process, another record is sent.
I think this can be fixed by reworking the comment submit form, in the
vein of my more recent form constructs, to remove the form as soon as
submit is clicked & display a message while the data is being handled.
I think this will take care of many of the multi-insert instances .

Bug description:
If you see the latest comments page, you'll notice the same exact
comment is getting to the database.
This could be a problem with how the comments are being handled with
aiki, or it coud be an aiki issue not checking the previous post for
rate limiting, or a bug.

It needs to be fixed.
There is some similar issue with the registration system on ocal,
openfontlibrary and in Aiki, allowing multiple entries of same content
into the db.
