jsc -> fs: the attached example document should contain a button in the top left
corner, or better it contains a form control when looking in the content.xml.
But the button is not visible. The attached document was created with a m73
office. But the original old sxw document, can't be loaded as well. The same
effect that the button is not visible.
The second attachemnt is only necessary to run the macro.
