The problem is fixed. When the replacement image is requested it is always
regenerated if the embedded document was modified after last image generation.
Of course it happens only in case of inplace/UI activation, in case of outplace
activation the image is changed only when the embedded document is stored or
closed.
For the send as email problem I have submitted a standalone issue 55222.
