The problem is still present on 1.9.130, on Windows XP.
Please do exactly as described in my initial comment:
"If the Open File dialog already has the Preview checkbox checked, the icon
image is
not displayed."
Note:
If the Open File dialog start display with Preview un-checked, the icon panel
will
appear if you check Preview. In this case choose an icon and OK. Immediately
after,
re-open the Open File dialog, and you are in the configuration I described 6
months
ago.
