Now trying to duplicate on a Linux machine, I was unable to confirm. However, there is a related problem in the importing of an Address book (both Linux and Windows). Take File>Wizards>Address Data Source... and try to create from a spreadsheet. The attached file shows the screen you get to assign fields. Note that you have "Street" here, and not "Address Line 1", as the Mail Merge Wizard requires. (If that weren't enough, from this screen it is impossible to assign the fields -- all those dropdowns are empty -- you have to go instead to File>Templates>Address Book Source..., where the dropdowns are correctly populated.)
