I would guess that what's going on is that you have no OpenType Hebrew fonts 
that have the built-in 
smarts (GPOS 'mark' tables, IIRC) to position vowel marks etc. Have you tried 
the test document with a real 
Hebrew font like SIL Hebrew, Cardo, or Ezra SIL?

I think was happening before is that when rendering with fonts that did not 
have vowel positioning 
information, ICU would use a heuristic method of guessing where vowel marks 
should go. A compromise 
might be to detect whether the font has 'mark' tables, and to use the heuristic 
method as a fallback if it 
doesn't.

Nevertheless, for accurate Hebrew rendering an OpenType font with vowel 
positioning tables should be 
used.
