At first, outline numbering may work as documented but quickly becomes locked.
That is, a heading 2 item will stay as “3.1” even if demoted to Heading 3 or
promoted to Heading 1. New headings and ordinary text will be given new numbers
in accordance with the immediately preceding line, irrespective of its correct
numbering according to outline level.

This has been reported many times and always seems to be marked as a duplicate
of 52888 but I don't think it is. 

Anyway, the cause is an incompatability between two features in OOO 2.0 as 
follows -

The default setting for AutoCorrect->Apply Numbering is ON. AutoCorrect
->ApplyNumbering detects that the previous line begins with a number and then
turns the Outline Numbering list into a Format->Bullets and Numbering List,
effectively turning OFF the outline numbering.
 
The Workaround is - 
Turn OFF AutoCorrect -> Apply Numbering in any document where you are using
Outline Numbering. With AutoCorrect -> Apply Numbering OFF, outline numbering
works as documented.

The Fix is -
1. Writer should automatically disable AutoCorrect -> Apply Numbering if the
user has applied Tools->Outline Numbering (because  AutoCorrect -> Apply
Numbering overrides/cancels the users preference)(Microsoft did this when they
introduced the same "feature" in Word 97 and it works well)
  
Perhaps there should be a once off dialog here advising the user that 
AutoCorrect -> Apply Numbering will be turned OFF to avoid conflicts with
Outline Numbering. This dialog would alert users to the existence of the
AutoCorrect -> Apply Numbering “Feature” and its incompatability with outline
numbering.

2. It's probably a good idea to turn  AutoCorrect -> Apply Numbering OFF by
default in the default templates supplied with Openoffice because, then if its
ON, its because the user has deliberately selected it for some purpose and is
aware that its ON. 

 3. Explain the incompatability of the options in both AutoCorrect -> Apply
Numbering and Tools->Outline Numbering sections in the help files.
