In steps 1 to 2 ... use current document, then 'letter' In step 3 - Click 'Select Different Addresses List' button - In the datasources list opt for 'create..' In the next dialog ..Choose customise... Delete all the fields one by one.[click delete continuously]..when all of them have been deleted click 'Add..' The input box for fieldname comes up.
