Do the crashed ones disappear or hang around? I've had this happen as well, but I'm not sure it's a OOo bug. I think it might just be that when an application crashes, Windows doesn't know to remove the icon from the task tray. Moving the mouse over it (for me) tells windows to update things and the extra quickstarters disappear.
