2006 -------
1. Open a template for editing
2. Insert, Fields, Other
3. Select "Date" on the Document tab
4. Scroll to Additional formats... and select. This opens the format dialog
5. Select a date format close to the format you want to create (e.g. 31 December
1999)
6. Edit in the Format code dialog (e.g. make it MMMM YYYY)
7. Click the check mark to add the format, click OK, then Close. Cancel the 
dialog
8 Save the template, then open a document based on it
9. Going to Insert, Fields, Other, the selecting "Date" does not show the format
you just added.
