The date field problem is already tracked as issue 29646.
Problem regarding filename field: generally filename fields in Word also only
support the format including the suffix ".doc". But here, the field content has
been changed manually. You can see this when opening the document in MS Word,
placing the cursor in the filename field and selecting "Update field" from
context menu. The field content would change back to "test.doc".
The possibility is also desired for the future in OO Writer, see issue 18779.
