Apply outline numbering to a document, and then save it. After re-opening then
select tools->Outline numbering and set the paragraphs style to "none" for the
applied outline levels. Then save the document and close it.
When reloaded, the first outline level will be a type different than "none",
thus breaking the outline structure.

Taking a first view at contents.xml, it seems that the text:outline-level
attribute of the original paragraph type is not cleared when setting the
paragraph style to "none".
