The problem is best viewed in the attached screenshot of the "Insert[sic] Index/Table" dialog, where both the "E#" and "E" for "Chapter no." and "Entry text", respectively, appear as "#F" and "F". Certainly, the buttons here are perhaps 1px too short, and this contributes to the confusion between "E" (in documentation) and "F" (viewed in dialog).
