I've seen this problem in OOo 2.0.4 and again in 2.1.0 on Windows XP.  The
problem is best viewed in the attached screenshot of the "Insert[sic]
Index/Table" dialog, where both the "E#" and "E" for "Chapter no." and "Entry
text", respectively, appear as "#F" and "F". Certainly, the buttons here are
perhaps 1px too short, and this contributes to the confusion between "E" (in
documentation) and "F" (viewed in dialog).

This dialog is accessed by either "Insert > Indexes and tables > Indexes and
tables", or by right-clicking an existing index or table and selecting "Edit
Index/Table".
