fixed in cws impress115 for OOo 2.2,

we really have to get rid of the hosemansche liste and page pointers....
Fixed three bugs actually. First, updated slides would not be replaced into
custom shows, I added this to SdDrawDocument::InsertBookmarkAsPage().
Second, slides that where removed from a document where also removed from all
custom shows, but only first occurrence. So if a slide was used more than once
in a custom show we had dangling pointers *sic*.
Last but not least, Thorsten fixed naming of inserted bookmarks, but he used
SdPage::GetName instead of SdPage::GetRealName. That lead to the following. If
the second slide is a linked one and has no name, it gets the generated name
"Slide 3", which is obviously wrong.
