Steps to recreate: 1) Create a slide in Impress. 2) Right click slide name in default tab, select "Rename" option and change the name, press ok. 3) Change tabs to "Notes". 4) Click into the text of the notes and/or type something. 5) Right click slide, select "Rename" option and change the name, press ok.
