Michael: the %%Title entry is used by print managers, too, to print the header
page (granted, most people turn that off, but it's a feature nonetheless).

So if UTF-7 and UTF-8 are off, how about this:
if the title does not contain non-ascii, use it as %%Title
else if the filename does not contain non-ascii, use it as %%Title
else emit no %%Title

(step 2 is not necessary with the viewers, but there may be other applications
that make use of %%Title)
