Too bad it isn't just an extension under the Index drop down. That seems too difficult to find. I added the screenshot so as you would know I found it.I went through the process of adding the icon for Form Controls to the Index drop down but it hasn't placed itself. I even restarted OOo 2.1 Writer and it still isn't in there. I then went under View>Toolbars> and added Form Controls and Index; which were previously unchecked. I still don't see a Command under Index for Form Controls. And, the toolbar for Form Controls that came up didn't have anything in it for inserting a text box.
