Ok, I downloaded the Crystal theme in question and made the zip archive
available for testing here:
http://people.bath.ac.uk/ea2aced/OOo/images_crystalX.zip

Looking through the archive, my optimized pngs have been included via fallback
to the Industrial theme. This work was integrated in CWS 'aceart02' for 2.2.
During QA, Petr noticed that some Patches in ooo-build disagreed with these
pngs. See later comments and screenshot for Issue 68830. Inactive icons were
being drawn in red due to the patch in Issue 35555. However, it was concluded:
"The new problem ... is ooo-build specific. It does not make sense to block
integration of this CWS."

Due to the timing of this bug and the red icons in 0emu0's screenshot (a defect
for sure), I suspect it is due to some interaction between ooo-build patch(es)
and the optimized icons. To test this, I have uploaded an old unoptimized set 
here:
http://people.bath.ac.uk/ea2aced/OOo/images_crystal204.zip
Please try replacing the current Crystal theme with this one and flushing the
icon cache.

Cannot recreate problem on WinXP with latest OOo build. Another thought, is
whether on different platforms the full feature set of png has not been
implemented causing certain problems?... 

Odd that the problem is not seen with the Industrial theme. Maybe due to using
Crystal with mixed in Industrial icons- does this cause some icons to be scaled?
