Test2.odt provides more hint to the problem... The problem appears when the
vertical text alignment of the table (or cell) is set to anything other than
Top. When file first opens, regardless of the table vertical text alignment
setting, the check boxes are aligned as if the table vertical text alignment is
set to top. After performing preview, the check boxs are realigned to their
proper placement.

In Test2.odt, I've created a table with various cases. Each cell shows the Table
vertical text alignment, and each check box is labeled with its own vertical
alignment (in the positions and size menu). The difference before and after
preview is very obvious.

Note that this affects the print and PDF output as well. I'm not sure whether
this problem affect other form controls as well, or is it isolated to check 
boxes.
