I found that setting the text:c attribute in content.xml to a very low figure,
such as 0 or 2, fixed the problematic document.  In the problematic paragraphs
that attribute was set to numbers in the 50s or 200s.

The customer of mine who is experiencing this issue has no technical skills
whatsoever, so the likelihood of them being able to fix the problem that has
affected their document without help is zero.  They were completely thrown by
this issue, believing that they couldn't add any further content to the 
document.
