I believe contents on a.m. page should be supplemented. Especially, if the user
tested with 2 "exotic" OS and has to select "All OS", you can not be sure that
the reported effect will appear for all OS, and so additional information
concerning the used OS will be useful.

Heading "OS: On which Operating System (OS) did you find this issue?"

Supplement: "In the 'Description' text, you should always mention the exact name
and version of the OS you used for your test, if it is not visible in field 'OS'
(for example, if you selected 'all').".
