Make the form controls toolbar visible ( View - Toolbars - Form Controls ) 3. Make the form design toolbar visible ( View - Toolbars - Form Design ) 4. Open a new Writer document ( Window - New Window ) 5. Use "View - Toolbars - Form Controls" to hide the form controls toolbar 6. Use "View - Toolbars - Form Design" to hide the form design toolbar 7.
