For the hid.lst: I do not know the complete process, but in general, there are .hid files generated during compiling resource files. This is for all UI elements which get automatic help ids (as is the case for most normal dialogs, controls, and the like, but for instance not for the generic Window or Control type). Additionally, projects may contain a hidother.src file, usually located in the util directory, which declares additional so-called "long names". This file is maintained manually. Somewhere in the build process (instsetoo_native, if I'm not wrong), those files are assmebled to hid.lst, and delivered into the environment. (side note: The fact that this assembling doesn't break when there are duplicate long names is to be considered a bug, IMO.)
