The CSV format mailmerge's address list creator writes to a file should have 
newlines at the end of every record and after the title record, but it does not.
hence it becomes unreadable to other applications like AbiWord, Excel, and 
possibly to mailmerge itself.

Tools|Mail merge wizard...
select current document
click next
click next
click address list...
click create...
