From what I can tell, the form wizard crashes at exactly the same point with an arbitrary database open. For example, one could simply open the second sample database I've attached, then start the form wizard. After selecting any set of fields, and choosing not to create a subform, the wizard crashes. I've tried this with both jdk1.5 and jdk1.6.
