This hang and the eventual crash is caused by two slide shows being created when
F5 is pressed only once.  One of the two is properly closed when the slide show
ends.  The other remains visible but is in a semi valid state similar to the
edit view.

Fixed by introducing a flag that prevents multiple instances of the slide show
for one document.  There was already code for this, but it did not cover the
start-up phase completely.
