Very recently I could reproduce the disappearance of embedded pictures in writer documents. That might lead as to the cause of the problem: The images disappear (sometimes all, sometimes only a part of them) as soon as my systems switches to energy-saving mode or is hibernated with the document still open. After awakening the system the pictures in the document have disappeared, first showing a frame with error message. After closing and reopening the document, the picture frame stays blank. Could you check, whether this is the reason to your problems as well.
