The attached document is indeed "ill". I can see that there is something wrong.
It is _not_ the mistake to use a list style. I cannot imagine why it is broken.
Do you remember the OOo version, which you used when starting the document or
the version in which the document template was build?

You have set your edit to record changes. This makes formatting more complex.
You should use this feature only if really necessary.

You can repair the document, if you reassign the paragraph styles "heading 1"...
Go to Edit > Find and search for the style "Heading 1" (expand dialog to mark
this feature). Use "Find all". Now all those paragraphs are marked. Use the
Style&Formatting window and assign the style "default" with a double click. The
former headings are still marked. Now immediately assign the original style
"Heading 1" with double click. Do so for the other heading styles too.
