Reopening, because I have a similar problem here in 3.0.1rc1.

How to reproduce:
1. Create a new text document
2. Add a header
3. Open "Data Sources" (F4)
4. Navigate to Bibliography->Tables->biblio
5. Drag and drop Identifier and Author fields into the header (<Identifier>
<Author>)
6. Drag and drop Identifier and Author fields into the text body (<Identifier>
<Author>)
7. Run Mail Merge Master
8. Go to the step 8 by pressing Next without any changes in steps' options
9. Choose "Save merged document" and "Save as single document" and save it

The result:
Header fields are processed only on the first page, while these fields are
processed as expected in text body on every page.

We have the same result if you choose "Save as individual documents" in step 8
of Mail Merge master.
