Just to summarize the information from the direct discussion with aviret:

- the lock file looks ok
- ';' and ',' symbols are not used in the user name
- the machine name is the same as the one mentioned in the lock file
- the tmp directory is available and writable
- it is enough to close the document and reopen it to allow further saving
- the used version is OOO300_m14
- there was a similar problem in OOo2.x

Based on the facts above it looks like the problem is not the locking mechanics.
I see currently two possibilities:
1) The document stream is hold somewhere, that prevents its closing. The stream
is opened using system file locking, as result the file can not be overwritten.
The probability of it is quiet small, since there is no known scenario in
OOo3.0.1 where system file locking still would be used for documents.
2) One of the temporary files related to the storage is removed by the system,
as result the document is based on the storage that is not completely valid.
Thus the saving might fail. Herbert has already mentioned that such a temporary
directory cleaning mechanics is used on MacOSX, so it is a quite probable 
scenario.

In case it is the second case the following workaround should help. In
"Tools/Options/OpenOffice.org/Paths" tab-page please set "Temporary files" value
to any own previously created location. The office must be restarted after the
change.
