Though this is all about the index, here we have different issues...

a) is already tracked as issue 41002.
b) is fixed in newer build 680m91 (maybe also in 680m90).
c) it seems that the referred headings were once Level 2 (Heading 2 style),
which made the TOC entries are indented. When updating the TOC in Word, these
are not indented anymore.
d) is similar to c). The Headings seem to be once written like this. Update the
TOC in Word and the entries will have capital letters like in Writer.

MRU->RICH: please don't be irretated by closing this as "WONTFIX", but there's
no identifier to mark this appropriate.
